IDD Medicaid rates in Indiana swing further from service to service than most readers would expect. Indiana pays $26.14 per 15 minutes for community integration on code 97537, or $104.56 an hour. That ranks 2nd of 22 states and is 291.6% above the national median of $26.70.
The same state pays $6.54 an hour for day habilitation on T2020, the lowest of 32 ranked states and 71.7% below the median. Its prevocational rate is second-lowest of 28. For an Indiana day services provider, the choice between billing community integration and facility-based day habilitation is the single largest driver of revenue. Rates are compiled from official state Medicaid publications and link to their source documents.

Residential habilitation on T2016 pays $280.27 a day, exactly the national median, 16th of 31 states. Indiana also publishes an ICF/IID rate of $367.53 a day, effective 2025-07-01.
Community integration on 97537: second-highest in the country
Indiana bills community integration and habilitation on 97537 with the UB modifier, effective since 2020-04-01. Only Virginia pays more, at $140.64. Indiana is ahead of Maryland ($68.44), the District of Columbia ($51.64) and Delaware ($39.24), and far above Wyoming ($16.60), Louisiana ($16.00) and Georgia ($14.60).
The margin is very large. Indiana's median direct support professional wage was $21.42 an hour in May 2025 federal data. Loaded with payroll taxes and benefits, that wage takes 28.3% of the 97537 rate, leaving $74.94 an hour. No other Indiana IDD service comes close.
Coverage records show 11 prior authorization rules on 97537, so the high rate applies only within authorized hours.
Day habilitation and prevocational: the bottom of the country
Day habilitation on T2020 pays $6.54 an hour, below Louisiana ($9.92) and Texas ($10.98), the other two lowest states. Minnesota, the highest, pays $100.00 an hour. Prevocational services on T2015 pay $5.66 an hour, above only the District of Columbia ($3.95).
These rates are group rates, with one staff member supporting several participants. Even so, a $6.54 hourly rate per participant leaves little room for staffing at a $21.42 wage. T2020 carries 33 prior authorization rules and T2015 carries 19, among the heaviest authorization loads on Indiana's IDD schedule.
In-home supports, supported employment and behavior support
Community living supports on 97535 with the U7 modifier pay $9.45 per 15 minutes, or $37.80 an hour, 8.2% above the $34.92 median. The loaded wage takes 78.4% of that rate, leaving $8.18 an hour. Indiana bills this service on 97535 rather than the T2017 used in many states.
Supported employment on H2023 pays $11.91 per 15 minutes, or $47.64 an hour, 16.7% above the median. The loaded wage takes 62.2% of the rate, leaving $18.02 an hour.
Behavior support on H0004 pays $18.56 per 15 minutes, or $74.24 an hour, effective 2026-01-01, 4.9% below the $78.07 median. H0004 carries 40 prior authorization rules, the most of any Indiana IDD code, and T2016 residential habilitation carries 27.

Wages close to the national median mean Indiana's margins depend almost entirely on which services a provider bills.
CIH and Family Supports waiver amendments
Indiana delivers IDD services through the Community Integration and Habilitation (CIH) Waiver and the Family Supports Waiver (FSW), with Money Follows the Person tracks for each. Several changes are in progress:
- Waiver amendments, effective 2026-08-01: public notices dated 2026-02-11 proposed amendments to the Community Integration and Habilitation Waiver, the Indiana Family Supports Waiver, the Indiana Health and Wellness Waiver and the Traumatic Brain Injury Waiver.
- Selective contracting: a notice of amendment to the 1915(b)(4) waiver for selective contracting for the CIH and Family Supports waivers, also dated 2026-02-11.
- Earlier FSW amendment: a public notice dated 2025-07-09 with an effective date of 2025-12-31.
Selective contracting can limit which providers may deliver certain waiver services, so current CIH and FSW providers should review the amendment text closely.
