Steps

  1. Get an NPI and pick the enrollment type: sole proprietor, rendering, ordering/referring/prescribing, group, or facility
  2. Create an account and file the application in the MPATH Provider Services Portal
  3. Sign the Disclosures, Screening and Enrollment Requirements form and the Terms and Agreements form
  4. Upload or mail the supporting documents (use the Montana Provider Services mail cover sheet if mailing)
  5. Wait for the welcome letter from Montana Provider Relations, which confirms the taxonomy to bill under
  6. Primary care providers who want to be Passport to Health medical homes also sign the Passport agreement

The managed care model they describe is Passport to Health program is primary care case management (PCCM) under a 1915(b) waiver: primary care providers sign a Passport agreement with the state and get a small monthly case-management fee per member.
